I acknowledge that I have downloaded the client eSignature copy of my prepared tax return(s) and related forms.

I acknowledge that I have downloaded all other supporting and prepared forms that were sent to me through a secure and encrypted portal.

I acknowledge that I have downloaded and reviewed my detailed invoice for the "Document Preparation Services" I have received. 

I authorize Doc Preparation Inc and its subsidiaries; not limited to Doc Prep Of Nevada, J1 Tax Docs, Doc Preparation Inc, to charge my credit card and or Debit Card for the amount shown on the invoice I received. 

I authorize my card to be charged as one time transaction per invoice. 

I understand that my card information will be saved to my secure and encrypted client profile. 

I understand and acknowledge that an additional electronic transaction fee of $10.00 will apply per card transaction; and that I have been given the option to pay thru ZELLE to avoid any transaction fees.

I acknowledge that a $25 Non-Sufficient-Funds Fee (NFS) will apply if my card is ran 2 times, and is rejected a second time, which is due to not having the funds available in my account at the time it is being charged. 

I acknowledge that if my card is rejected a second time, I will be required to pay using a different payment method and that my tax return will not be filed with the IRS until my full payment is rendered.
